Below you will find forms that are needed to register for the 2019 season! There are 3 ways to register this season.

First way is to print the forms and send in with payment to P.O Box 377 Ashley, Ohio 43003. Please provide an email address as this is where your receipt will go to.

Second way is to Fill out forms online and submit online.

Third way is to come in and see us on Pancake day at Buckeye Valley High School from 7:30 am until 12:30pm. Date March 2, 2019

** Refunds in full will be before deadline of March 9, 2018 @ 11am when rosters get turned in and uniforms are ordered. Can still add to rosters until March 21st. Refunds after this date are minus uniform or products bought (i.e socks, belt and so on).There will be NO refunds after season fees are paid to league.**
